This is the application for the DeVry University Passport2College program that will be held at the Houston, Texas main campus and Galleria center during the month of June. A special enrollment application will be required for those students accepted into the program. Applicants who are not 18 years of age will need the signature of a parent or legal guardian.

Deadline for Application:

This application must be submitted by May 29, 2009. DeVry University reserves the right to limit class size. Enrollment is on a first-come, first-served basis for those who have completed the application process and met the application deadline.
