Registrar and Program Coordinators
The Registrar is responsible for student records, transcripts, transfer of prior college credit, and changes of names, addresses, and phone numbers. Program Coordinators are responsible for monitoring attendance and academic advising.
The Registrar's Office prepares letters of verification of enrollment. These verifications may be used for loan deferments and "good student" auto insurance discounts. Please allow 48 hours for preparation.
Copies of curriculum records (degree plans) listing all required courses and whether accounted for or not may be obtained in the Registrar's Office.
Transcripts
Individual academic transcripts are available from the Registrar's Office. Graduates receive one complete validated transcript at no charge. All other transcript requests will be $5.00 per copy. There is a 48-72 hour wait to receive transcripts after being requested. Transcripts will not be issued until all obligations to DeVry have been met.
Course Audit
Students may audit courses by declaring their intentions to the Registrar before the end of the second week of instruction. Normal tuition is charged and financial aid is not applicable.
Audited courses do not fulfill degree requirements. Evaluation and class participation are optional; however, regular attendance is required.
Procedure for Name and Gender Changes to Student Education Records
DeVry University adheres to the standards set forth by AACRAO regarding name and gender changes. The following procedure standardizes the process and identifies appropriate documentation students must submit for a name or gender change (other than those changes resulting from an oversight by staff entering the application) to their academic record. Students' Responsibilities
* Students must submit the signed and dated Change of Personal Information form available through the registrar's office and online at the campus community website.
* In addition to the form, students must submit accompanying official/legal evidence such as a copy of a driver's license, marriage certificate, divorce decree, adoption papers, passport/permanent visa or court order reflecting their new name/gender; an official court order affirming a gender change or a driver's license is required for a gender change.
* If students do not submit the Change of Personal Information form, their request must be made in writing, dated, signed by the student and contain the following information:
o Student ID Number
o Date of birth
o Current term of enrollment or last term attended at DeVry University or Keller Graduate School of Management
o Former name (or gender) appearing on the education record
o Current name (or gender) as requested to appear on the education record
o Copy of official/legal document such as the following: a driver's license, marriage certificate, divorce decree, adoption papers, passport/permanent visa or court order reflecting the new name; an official court order affirming a gender change or a driver's license is required for a gender change
* Students may fax, mail or submit in person their written request/form and accompanying legal document to the campus/center location at which they are currently enrolled or last attended.
Registrar's Responsibilities
* Make the form available at the Registrar's office and online at the Registrar's web pages housed on the campus community website.
* Ensure that appropriate supporting documentation has been submitted and that the form is signed and dated by the student.
* Ensure that the documentation type that student submitted is indicated on the Change of Personal Information form.
* Enter the change into the student information system; if the student has progressed in the current system, run the degree audit to push the name change to Degree Navigator.
* Shred the supporting documents and scan the form as part of student's permanent academic record; if the student's file has not been previously imaged, place a copy of the form in the student's folder maintained at the campus and/or center.
* Notify the student's home center and previously attended campus of the change, if applicable, to ensure communication of the name change is shared between all parties maintaining student records.
